Use the form below to make a formal complaint to MediaWorks about a television programme on TV3, FOUR, C4, or radio broadcast on Mai FM, The Edge, George FM, The Rock FM, MORE FM, The Breeze, The Sound, RadioLIVE, LiveSPORT, Kiwi FM.
Formal complaints must be received by MediaWorks within 20 working days of the Television programme or Radio broadcast.

With the exception of complaints about breaches of privacy (which may be sent directly to the Broadcasting Standards Authority at PO Box 9213, Wellington), you must send your complaint to the broadcaster in the first instance.
